Flourish and Flow is committed to providing quality services to you. This policy outlines our ongoing obligations to you in respect of how we manage the personal information we collect, use and disclose. Flourish and Flow complies with the Privacy Act 1988, as well as other relevant State and Territory legislation regarding the management of personal information.

What information do we need to collect?

Whilst this list is not exhaustive, we may request to collect the following:

  • Personal information such as your name, date of birth, family members’ names, home address, email address, phone numbers and relevant funding information.

  • Diagnostic information such as medical and assessment reports, as well as letters from doctors and other health care professionals.

  • A signed consent form and service agreement.

Why do we need to collect your personal information, and how will it be used?

The process of collection and management of personal information is in alignment with the Australian Association of Social Worker’s Code of Ethics, where it will be treated as confidential in the course of professional services. We collect personal information in order to deliver high quality services. This information enhances the coaching and counselling interventions provided. Additionally, it is used for administrative purposes, such as sending an invoice to you, your insurer, compensation agency or NDIS (if applicable). If you receive NDIS funding, we will also need to collect relevant funding information in order to meet reporting requirements. 

When might information need to be disclosed with others?

With your consent, Flourish and Flow may disclose relevant information pertaining to assessment, diagnosis and treatment to your other services or health professionals. Should you have any queries or concerns regarding our handling of your personal information, please do not hesitate to raise these with us.

Flourish and Flow will only disclose personal information in accordance with the Privacy Amendment (Private Sector) Act 2000. This means it will only be disclosed if:

  • We have your consent; or

  • We are required by law to disclose it; or

  • It is otherwise permitted under the Act.

How is your information secured?

Flourish and Flow will take reasonable steps to secure any personal information which we hold. Hard copy personal information is stored in a secure and locked filing cabinet, and electronic information is on a password protected computer to be accessed only by Flourish and Flow staff members. Flourish and Flow takes all reasonable steps to ensure that information collected about you is accurate, complete and up-to-date.

Access of Information

You may have access to your information on request and if you believe that any of the information is inaccurate we may be able to amend it accordingly.
